MEDIA RELEASE FORM - Must Read and Sign
This release form (“Release”) pertains to the artwork, rights of publicity, name, logo, images, audio and visual recordings, and customer stories to be utilized by Savvy Cleaner Training, LLC. ("Savvy Cleaner") (such materials, the “Materials”).

I represent that I am the owner and/or authorized representative of the Materials and that I have the authority to grant you the permission and rights herein granted, and that no one else’s permission is required with respect to the rights herein granted.

For good and valuable consideration, receipt of which is hereby acknowledged, I, the undersigned, grant to Savvy Cleaner, its agents, successors, licensees, and assigns, the non-exclusive right but not the obligation to use the Materials and to utilize and reproduce the Materials throughout the world, in any and all manner, methods and media, whether now known or hereafter known or devised and in the advertising, publicizing, promotion, and exploitation thereof. I understand the Materials will be used in connection with marketing, publicizing, and promoting Savvy Cleaner and its services, as well as being featured in advertising and promotions, either by Savvy Cleaner or in conjunction with third parties (collectively, Savvy Cleaner, and any third parties are the “Entities”).

I hereby irrevocably authorize the Entities to copy, exhibit, sub license, publish or distribute the Materials in complete or edited form, for purposes of publicizing Savvy Cleaner and its services, or for any other lawful purpose throughout the world. The Materials may be used in printed publications, multimedia presentations, television commercials, websites including social media platforms such as YouTube, Instagram, Facebook, Twitter, Vine, and Pinterest, or in any other distribution media. I agree that I will make no monetary or other claims against any of the Entities for the use of the Materials. In addition, I waive any right to inspect or approve the finished product, including a written copy or edited video wherein my likeness or the Materials appear.

I hereby hold harmless and release the Entities, and their successors and assigns, from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
